During the third Scarecrow nightmare the picture stutters and freezes as if the console has crashed, then the game appears to restart with its opening scene, except now the Joker is driving Batman into the asylum. It ends with an on-screen prompt to dodge using a "middle stick" that no controller has. Many players reached for the power button, which is the joke.
How to find it
Play the story until the third Scarecrow fear-toxin sequence; the fake crash happens on its own.
